  • Conducting Addiction Education Seminars.
  • Assisting and educating patients' families.
  • Following-up to keep patient focused and goal oriented as clinically and medically determined.
  • Working with a physician on development of treatment plans.
  • Continuity of Care, which could include assisting patients with necessary care related to medical, housing, employment, transportation, etc.
  • Community Outreach with hospitals, residential facilities.
  • Peer to Peer interaction with Social Workers, IOP Addiction Counselors, Care Coordinators, in the local community.
  • Presentations and outreach to ER departments, Sub-committees and community influencers introducing our services and getting our program recognized.
CleanSlate is seeking a Licensed Clinical Social Worker (LCSW) for our Tampa, FL center, with experience in addiction, who lives and works in the Tampa Bay area. Must possess a strong desire to work with patients who are battling opioid addiction. Individuals must have excellent patient care, be strong communicators, and possess documentation and organizational skills. This is a visible role in the company and will encompass Community Outreach and Patient Care.
